This is my registration for the Log Cabin Chinking and Daubing Workshop sponsored by the Friends of the Log Cabins Association, to be held in the Lincoln-era Log Cabin Village located in the Quinsippi Island Park, Quincy, IL starting at 12 PM, Monday, April 4th ending at 12 PM Friday April 8th, 2022.
Enclosed is my registration fee of $125.00, which includes a box lunch for Tuesday, Wednesday and Thursday.

Note: Instructor requests proof of Vaccination for COVID to include Booster shot. I understand there is a requirement for proof of COVID vaccination to include the Booster shot. I will provide that proof on the first day of the workshop.

I understand if I cancel before March 11th, I will receive a refund of my full registration fee (if paid), but after that date I will not receive a refund.
I understand registrations must be received by March 11, 2022. If too few registrations are received, I understand the workshop may be canceled and if so, I will receive a full refund.
I understand I am to have my own insurance and will not hold the Friends of the Log Cabins Association liable for injuries incurred in participating in this workshop.
